Elara Caring is looking for a passionate RN Case Manager to join our elite team of healthcare professionals and make a difference, one patient at a time.

Schedule: Monday through Friday, full-time

Key Responsibilities:

  • Assess and identify the physical, psychosocial, and environmental needs of hospice patients.

  • Develop and implement personalized care plans.

  • Conduct initial and follow-up evaluations to ensure the highest standards of care.

  • Communicate significant findings and changes in patient health to the care team.

  • Educate patients, families, and care staff on hospice philosophy and pain management.

  • Provide support at the time of death and perform bereavement assessments.

  • Maintain patient confidentiality and adhere to HIPAA regulations.

What is Required?

  • Positive attitude

  • Strong communication skills

  • Reliable transportation to perform job duties

  • Advantage Program Certificate, where state applicable

  • Bachelor’s degree required, with additional one year paid professional experience, or one year as a Registered Nurse or Licensed Practical Nurse in lieu of an Bachelor’s degree.

  • Preferred to Home Health or Hospice experience

  • 50% travel
