As Clinical Team Coordinator, you’ll contribute to our success in the following ways:
Well versed in hospice workflow and regulatory requirements to drive quality measures and proper documentation of eligibility.
Expertise in EMR and Hospice platforms (SHP)
Assures hospice regulatory compliance through review of hospice documentation.
Processes hospice workflow, including but not limited to admission, discharge, and HIS workflow.
Provides feedback and coaching using standard Elara resources.
Communicates effectively with all members of the interdisciplinary team.
Monitors for trends when completing workflow and involves hospice leadership and education team to provide support to clinical team members.
Promotes Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care.
Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
Performs other duties/projects as assigned.
What is Required?
Associate or bachelor’s Degree and graduate of an accredited program of nursing.
Current license as a Registered Nurse
5+ years’ experience as a Registered Nurse in a Hospice environment
Experience with Hospice quality activities is preferred.
Must have dependable vehicle, valid driver’s license, and current auto insurance in accordance with the laws of the state.
Ability and willingness to travel approximately 10% to meet business needs.
You will report to the VP of Clinical Services.
