As Orders Coordinator, you’ll contribute to our success in the following ways:

  • Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Order and Claims Management team.

  • Maintains patient medical records in HomeCare HomeBase (HCHB) and Forcura.

  • Processes and sends all orders to the appropriate physician or clinical provider for signature, including but not limited to Plan of Care/485, Verbal Orders, Add-on Evaluations, Discharge, Follow-up, Resumption of Care, Transfer to Inpatient Facility.

  • Monitors all documentation folders in Forcura and follows up appropriately.

  • Coordinates with branch staff to engage clinical providers, procure order signatures, and coordinate documentation needs, as needed.

  • Coordinates with sales staff to procure order signatures and communicate with providers, as needed.

  • Oversees the appropriate use of agency resources including, but not limited to, supplies and personnel to ensure financial success.

  • Works to maintain positive, collaborative relationships with referral sources, signing physicians, primary care providers, patients and their caregivers, and internal business partners.

  • Participates in internal staff meetings and training activities, as assigned

  • Promotes Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care.

  • Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.

  • Performs other duties/projects as assigned.

What is Required?

  • High School Diploma/GED is required

  • 3+ years’ experience in a health care setting preferred

  • Working knowledge of Medical Terminology preferred

  • Excellent customer service skills are required

  • Proficiency with MS office Suite (Word and Excel) is required

You will report to the Regional Orders Manager.
